As I set to work on this new edition of Psychodynamic Psychiatry in Clinical Practice, I realized how much had happened in the field since the previous version of the textbook. I found myself in something of a quandary. To keep the price affordable, my colleagues at American Psychiatric Publishing wanted me to approximate the overall length of the third edition. I could not add all the new material I wanted to include without greatly expanding the size, weight, and cost of the book. Thus, I had to systematically read through each chapter and decide what parts of the text could be sacrificed without compromising the comprehensiveness and usefulness of the volume. In each case I looked for research findings or antiquated theoretical formulations that no longer qualified as "cutting-edge" material. The task was challenging because at the same time I sought to preserve clinical wisdom that was essentially timeless and had accumulated over decades of practice.
